The goal of the SCC Program is to provide the unique health and safety supports necessary for each person so that they can live a happy and meaningful life as part of their greater community.

Supported Community Connections (SCC) is a program designed to enable individuals to lead happy and meaningful lives as valued members of their broader community. As part of PDI’s Day Habilitation services, SCC offers personalized 1:1 and small group activities exclusively held in community settings. The activities are tailored to each individual’s unique interests, allowing them to choose the peers they wish to engage with, the locations they visit, and the frequency of their participation in the program. Popular activities selected by participants include outings such as bowling, zoo visits, gym sessions, fishing trips, swimming excursions, attending community events like baseball games, the CO State Fair, rodeos, and much more.
