H.O.P.E. (Helping Our Partners Engage) is a family-centered program designed to help families of youth with intellectual and developmental disabilities begin planning early for a successful transition into adulthood. By connecting families with information, resources, educational opportunities, and community partners, H.O.P.E. helps create a roadmap for the future long before important decisions need to be made.
We believe that when families start planning early, individuals with diversabilities have greater opportunities to explore their interests, build independence, develop skills, and pursue meaningful lives within their communities.
Through workshops, family events, peer connections, and partnerships with schools and service providers, H.O.P.E. empowers families with the knowledge and confidence needed to prepare for the future at every stage of the journey.
At Pueblo Diversified Industries, we believe planning should begin long before adulthood arrives. H.O.P.E. helps families turn uncertainty into possibility by providing the tools, connections, and support needed to create a future filled with opportunity, independence, and purpose.






For students aged 18–21 with IEPs. Supports advocacy, meetings, and goal alignment. Helps families navigate services for independence.
Starts in middle school with coordinated planning. Emphasizes collaboration across agencies and IEP goals. Ensures access to needed services into adulthood.
Encourages supported decision-making when possible. Promotes independence while honoring individual needs. Offers alternatives to guardianship when available.
1. Supported Living Services (SLS) Waiver
For adults (18+) with IDD who do not require 24/7 care. No waitlist. Services include: Personal Care, Homemaker Assistance, Mentorship, Respite, Day Programs, and Supported Employment.
2. Developmental Disabilities (DD) Waiver
For adults with IDD who need 24/7 care. Waitlist applies.
Services include: Residential support (Group Homes, PCAs, Host Homes), intensive habilitation, and employment services.
