FOR FAMILIES
PDI’s H.O.P.E. Program supports families of youth with Intellectual and Developmental Disabilities (IDD) as they navigate the transition from high school into adulthood. The program provides education, guidance, and practical tools to help families understand transition planning, adult services, and Medicaid-funded waivers.
By bringing together families, schools, and community partners, H.O.P.E. helps create a clear, informed path forward—so individuals and their support networks feel confident and prepared for life beyond high school.






For students aged 18–21 with IEPs. Supports advocacy, meetings, and goal alignment. Helps families navigate services for independence.
Starts in middle school with coordinated planning. Emphasizes collaboration across agencies and IEP goals. Ensures access to needed services into adulthood.
Encourages supported decision-making when possible. Promotes independence while honoring individual needs. Offers alternatives to guardianship when available.
1. Supported Living Services (SLS) Waiver
For adults (18+) with IDD who do not require 24/7 care. No waitlist. Services include: Personal Care, Homemaker Assistance, Mentorship, Respite, Day Programs, and Supported Employment.
2. Developmental Disabilities (DD) Waiver
For adults with IDD who need 24/7 care. Waitlist applies.
Services include: Residential support (Group Homes, PCAs, Host Homes), intensive habilitation, and employment services.
