

PDI’s Benefits Planning Department empowers people with disabilities and their families to navigate complex benefit systems with confidence. Our Cornell University trained planners provide expert guidance to help you maintain financial stability, explore employment opportunities, and navigate life transitions.

PDI’s Day Program gives people the chance to connect with peers, build skills, and explore their community in a supportive environment. Participants engage in activities—both on-site and off-site—based on their interests, from cooking and gardening to art, exercise, and social networking.

PDI’s H.O.P.E. (Helping Our Partners Engage) program is designed to empower families of youth with Intellectual and Developmental Disabilities (IDD) by guiding them through the complexities of transition planning and accessing Medicaid-funded services (waivers).

PDI’s residential services are designed to balance maximum independence with quality care. Through our network of Host Homes, people live with caring families who provide a safe, supportive, and engaging home life.

The Supported Community Connections (SCC) Program helps people live happy, meaningful lives as valued members of their community. Part of Human Service Programs, SCC offers personalized 1:1 and small group activities—always held in community settings.

Supported Living Services (SLS) are designed for people who want extra support at home—whether they live with family or independently. As a Class B licensed provider, PDI offers a range of SLS options through Colorado’s Community First Choice (CFC) program, delivered by trained staff. Services may include assistance with personal care, homemaker skills, money management, mentorship, and respite care when needed.

PDI’s Pre-Vocational Program prepares people for meaningful employment through personalized skill-building and support. Training covers work habits, communication, decision-making, problem-solving, safety, and self-advocacy—removing barriers to workplace success.
