FOR FAMILIES

Supported Living Services

Supported Living Services (SLS) are designed for people who want extra support at home—whether they live with family or independently. As a Class B licensed provider, PDI delivers a variety of SLS options through Colorado’s Community First Choice (CFC) program, provided by trained staff. Services include assistance with personal care, homemaker skills, money management, mentorship, and respite care when needed.

Our team can assist with personal care, provide transportation, manage medical needs and medications, and support people in building independence and learning new life skills. We also offer emotional encouragement and guidance during challenging times, helping people feel supported every step of the way.

Becoming a Family Caregiver within the Supported Living Services program is both a meaningful and rewarding role. If this feels like a path you’d like to explore, we invite you to contact us to learn more or to discuss any questions you may have.
