FOR FAMILIES
Supported Living Services (SLS) are designed for people who want extra support at home—whether they live with family or independently. As a Class B licensed provider, PDI delivers a variety of SLS options through Colorado’s Community First Choice (CFC) program, provided by trained staff. Services include assistance with personal care, homemaker skills, money management, mentorship, and respite care when needed.
Our team can assist with personal care, provide transportation, manage medical needs and medications, and support people in building independence and learning new life skills. We also offer emotional encouragement and guidance during challenging times, helping people feel supported every step of the way.

Assistance at home to meet physical and supportive needs, either hands-on or through guidance.

Basic or enhanced household support to maintain a safe, healthy home includes: shopping, and meal prep.

Guidance in essential independent living skills like money management, safety, and social boundaries.

Hourly care to give primary caregivers a break, provided at home or in the community.
Starts in middle school; emphasizes collaborative inter-agency planning, IEPs, and access to services through adulthood.
Encourages supported decision-making over guardianship when possible.
